All BC/BS, Preferred Care, MVP and Medicare plans will be accepted. 

Please click on the appropriate link below to see if your plan covers the flu vaccination in full.  If your plan is not on these lists, you are responsible for any co-payments and will be billed. 

For all other insurance plans there is a $30 on site vaccination charge.  We accept only cash or checks made out to “Health Checkpoint”.
